  2. Select location, date, and time for your procedure →
  3. Complete payment of $ 100 deposit (required)
  4. Fill out personal information and sign the electronic consent form

Have your insurance information available (if applicable). Verify coverage with your insurance company.

 

Deposit Policy:

  • Deposit applied towards the cost of the procedure
  • Refundable until 7 calendar days prior to the procedure date
  • Not refundable thereafter for cancellation or postponement (special circumstances may be considered)
